The transition from hospital to a rehabilitation center can be a stressful process. There are so many decisions to make and information to pull together. If you have any questions or need help taking the next step we are just a phone call away.

Embracing the Journey means that we are not only committed to caring for you while you are with us;we offer support even after you leave Blue Lake Post Acute.
Our discharge team can coordinate services for return home or back to the community. This may include scheduling follow up appointments with the primary physician.
We are able to assist with durable medical equipment that may be needed post-discharge.
Based on the individual care needs after discharge they may have home health or home care services arranged.
Through pharmacy provider; we can assist with the post discharge pharmacy overview ( the pharmacist will follow up after discharge to assist with any medication concerns to help prevent re-hospitalizations).
